How we use it
We use this information to respond to your request, schedule care, operate and improve the site, measure advertising, and meet legal obligations.
How we share it
We share information with service providers that help us run the site. The site is hosted on Netlify. Forms are processed by Netlify Forms. Analytics are provided by Google. We use the Meta Pixel to measure and improve our advertising on Meta platforms. It collects page visits and whether an appointment request was submitted, but not the contents of forms. You can opt out in Meta's ad preferences. We may also share information when the law requires it.
We do not sell personal information.
Cookies and analytics
We use cookies for analytics and to measure advertising. The site uses Google Analytics 4, measurement ID G-8KC3YFT2S9. Google Signals and ad personalization are turned off. Google Analytics 4 does not store your IP address. We do not send the contents of form fields to analytics or to the Meta Pixel. We may use Google Ads conversion measurement.
We use the Meta Pixel to measure and improve our advertising on Meta platforms. It collects page visits and whether an appointment request was submitted, but not the contents of forms. You can opt out in Meta's ad preferences.
You can opt out of Google Analytics with the Google Analytics opt-out browser add-on.
Data retention
We keep form submissions and related records for as long as we need them to respond to you, run the practice, and meet legal obligations.
Security
We use reasonable safeguards to protect information. No method of sending or storing information on the internet is 100% secure.
HIPAA
This policy covers how you use the website. Once you become a patient, your protected health information is governed by our separate Notice of Privacy Practices. That notice is available at the office or on request.
Do not send emergency or sensitive medical information through this site.
Your choices
You may ask us to access, correct, or delete personal information you have sent through the site. Contact us at the phone number or email above.
If you are a Texas resident, you may contact us to ask about personal information collected through this website.
Children
This site is not intended for children under 13. We do not knowingly collect personal information from children under 13.
Changes
